The TP5100 I used to replace the guts of my charger (entirely based on Jason's webpage) accepts 5V, is dirt cheap, and has resistors you can add/remove/change to adjust the current. I don't have a suggestion for a case, as I've never liked HT cases. They do protect the radio, but make it more annoying to use. I use a Kenwood SMKW1002 (google for that and you'll find it) which is a very sturdy button that attaches to the radio and a clip for your belt. I have three of the button...

By the way, I got an MD-760 battery eliminator a week or so ago here and it fits perfect... You are correct, scan dwell time is the amount of time the radio listens to a channel before it decides that there is a signal or not. 30ms is the default and gives the fastest scanning speed, this has been found to work reliably on most radios. However it may be that your radio needs a little long...
